As public health officer Do Ji-ui (Lee Jae-wook) entered Pyeondong-do and met Yuk Ha-ri (Shin Ye-eun) for the first time, various hardships and adversities unfolded.
Do Ji-ui, who was performing his national defense duties as a replacement service worker, was struck by the sudden news of his assignment to Pyeondong-do, but entering Pyeondong-do was not smooth. He fell into the sea while trying to save Yuk Ha-ri, who was standing precariously on the deck.
Afterward, Do Ji-ui discovered targeted anticancer drugs in a swapped bag and realized why Yuk Ha-ri had looked so sad, which made him uneasy. However, contrary to Do Ji-ui's memory, the truth was that Yuk Ha-ri had carried Do Ji-ui, who was seeing hallucinations due to being intoxicated by the medicine.
Do Ji-ui's vision went dark upon seeing the old medical equipment at the Pyeondong health center and the insects filling the official residence. The first patient, Park Chun-sik (Woo Hyun), claimed to have simple indigestion and left, only to be found collapsed later, putting Do Ji-ui in a difficult position.
After performing CPR and transferring him to the mainland by helicopter, the crisis was averted, but Do Ji-ui faced the obstacle of having to return to Pyeondong-do by boat. He had a trauma regarding the sea. At that moment, Yuk Ha-ri placed headphones on him and instructed him to calm his mind.
As Do Ji-ui gazed intently at Yuk Ha-ri, he recalled the warning: "The three things to avoid on an island: people, incidents, and love."
